You can email, mail or bring it with you when you pick-up your order. We are asking you to complete pages 1-3 to register for the VIP Club. The rest of the form will help us serve more of your needs. It is your choice if you want to complete them. The survey is always helpful to us for future product inventory acquisitions.
The cost is $12 a year payable at the time of your first order.
